At First Baptist Church we are committed to the health and safety of your children. Therefore, it is mandatory that every child, student, or youth under the age of 21 participating in a First Baptist Church sponsored trip, activity, or special event have an updated medical release form on file. This form must be updated every calendar year.

Please note that this form must be notarized. There is a notary at the church office Monday-Friday if needed.
